Students who need certification that they have graduated from the College of Letters & Science at the University of Wisconsin-Madison can obtain assistance from the Office of the Registrar. This unit can assist students in filling out the Dean’s Form required by US law schools, medical schools, and graduate programs during the application process. Please make sure you provide your full name and UW-Madison student ID number when communicating with this unit.
The Office of the Registrar also provides information regarding disciplinary clearance that may be required by graduate, law, medical and professional schools. Additional information about disciplinary clearance can be found at dean’s certification on the Dean of Students Office website.

Diplomas are usually mailed approximately four (4) weeks after L&S has notified the Office of the Registrar’s Office the names of students who have completed all of their degree requirements. Diplomas will not be mailed out to students if they still have certain types of holds on their academic records. Once the holds have been cleared, students’ diplomas will then be automatically released for mailing. See diplomas for more information.
